Your choices can be expressed in advance, before you are unable to express them due to progression of an illness or other unanticipated problems.
Available Advance Directives
An advance health care directive allows you to state your desires around medical care and to select a trusted person to make decisions for you in case you are unable to do so. If you already have an advance health care directive, please bring it with you when youâre admitted to the hospital.
