Community Advisory Board
The Community Advisory Board, along with hospital care coordinators, partners with organizations that support patients from UCSF Health Hyde Hospital and UCSF Health Stanyan Hospital – especially those patients with the highest need. Together, we develop community-based partnerships to address long-term care and gaps in the health system.
